Made Before the Teeth Come Out

An immediate denture is the one complete denture that has to be finished before you can see the mouth it will sit in. The teeth are still there when we build it, and it is delivered the day they come out — so the patient is never without teeth.

We fabricate it from a pre-extraction record, then modify the cast to approximate what the surgeon will leave behind. On delivery it also acts as a dressing over the extraction sites.

  • No wax try-in: The natural teeth still occupy the space, so esthetics are set from records and photos, not confirmed in the mouth.

  • Reline is planned in: The ridge resorbs through healing, so a reline — or a definitive denture — belongs in the plan from day one.

Indicated for

  • Patients transitioning to complete dentures who cannot be without teeth after extraction
  • Full-arch extraction cases where the denture is delivered the same day as surgery
  • Situations where maintaining esthetics and function through healing is a treatment requirement
  • Cases where the extraction sites benefit from the coverage an immediate denture provides
  • Patients staging toward a definitive conventional or implant-supported prosthesis

Consider an alternative when

  • The teeth are already extracted and the ridge has healed — a conventional or digital denture is the direct route
  • The patient can accept an edentulous healing period, allowing a definitive denture on a settled ridge
  • Only a few teeth are being replaced — an immediate acrylic partial or flipper fits the case
  • The definitive plan is implant-supported and the sequence allows a different provisional strategy

Immediate Dentures — Questions Dentists Ask

We lean hard on pre-op photos, the existing tooth position, and clear jaw relation data. The cast is prepared to approximate the ridge after surgery. Expect chairside adjustment at delivery as a normal part of the procedure — not a failure — and plan the reline into treatment from the beginning.

Pre-extraction impressions or scans, jaw relation with VDO and centric, tooth mold and shade, pre-op photographs, exactly which teeth are being extracted, and the surgical date. This case is scheduled backward from surgery, not a standard queue.

The ridge will resorb through healing. A reline or a definitive denture belongs in the plan, and the patient should hear that at the outset — otherwise the first soft-tissue change feels like a broken promise.
